The Ebola outbreak in the Democratic Republic of the Congo (DRC) has reached a critical juncture, with confirmed cases exceeding 4,000. Health officials are increasingly concerned that the Bundibugyo strain of the virus may be mutating complicating efforts to control the spread. The outbreak, first reported on 15 May has now become the second-largest Ebola outbreak on record, with 3,973 cases and 1,801 deaths recorded as of 4 August.

The situation has prompted the Africa Centres for Disease Control and Prevention (Africa CDC) to escalate their response. Dr. Jean Kaseya, Director General of Africa CDC, announced plans to intensify efforts, including door-to-door searches for patients. The outbreak, centered in the conflict-affected mining province of Ituri has also spread to Nord-KivuSud-KivuHaut-Uele and Tshopo.

Unprecedented severity and community transmission

The current outbreak’s severity is unprecedented, with more than two-thirds of Ebola deaths occurring in the community rather than in treatment centers. In a treatment center run by Médecins Sans Frontières (MSF) in Bunia 90% of admitted patients do not appear on authorities’ lists of contacts of known cases. This indicates high levels of untracked transmission.

Dr. Kaseya highlighted the insufficient contact tracing in the DRC, with only 10 contacts identified for every Ebola patient, far below the expected 40 contacts. The outbreak’s rapid spread has been exacerbated by logistical challenges and resource constraints, particularly in conflict-affected areas.

Response efforts and new strategies

In response to the escalating crisis, health officials are planning to shift from contact tracing to active case search. This involves community health workers moving from door to door to identify households with Ebola symptoms. Dr. Wessam Mankoula, acting head of emergency preparedness and response at Africa CDC, emphasized that the time for incremental action is over, and a scaled-up response is now necessary.

The planned response includes a village-centered approach involving communities, utilizing digital tools, and taking action in camps for internally displaced persons. Officials also plan to begin using the antiviral drug remdesivir on a compassionate use basis and testing the Ervebo vaccine licensed for a different strain of Ebola, in the affected provinces.

Vaccine development and therapeutic interventions

The urgent need for a vaccine against the Bundibugyo strain has spurred international efforts to develop and test candidate vaccines. The International AIDS Vaccine Initiative (IAVI) has developed a single-dose candidate using a recombinant vesicular stomatitis virus platform. Additionally, researchers are investigating adenovirus-vectored constructs and mRNA vaccine technology to diversify the preventive toolkit.

Collaborative networks, including Hilleman LaboratoriesMerck and Wellcome are leading manufacturing efforts. The Coalition for Epidemic Preparedness Innovations (CEPI) is providing multi-million dollar grants to support the rapid production of clinical-grade doses. The University of Oxford and the Serum Institute of India are also advancing separate vaccine candidates.

While preventive tools are being tested, clinicians are evaluating therapeutic interventions. Investigational antibody therapies and direct-acting antiviral medications are undergoing clinical evaluation in outbreak zones. Standardized supportive care remains the primary baseline intervention for patients, with fluid management and infection control measures being critical during hospitalization.

The current epidemic underscores the necessity for sustained biosecurity preparedness. International coalitions are transforming outbreak response paradigms, investing in versatile manufacturing platforms, and enhancing global health resilience. Cross-border surveillance networks and community engagement strategies are vital to foster public trust and timely containment measures.
