When we think about the health risks of extreme weather, we often focus on heat stroke, dehydration, or respiratory issues. However, there’s another significant concern that affects millions of older Americans: chronic pain.
In a comprehensive study led by public health professor Kai Zhang researchers tracked over 35,000 adults aged 50 and above for two decades. They examined how prolonged exposure to extreme heat or cold influences the prevalence and severity of chronic pain.
The impact of extreme temperatures on chronic pain
The study defined high-impact chronic pain as pain lasting at least three months that interferes with daily activities such as work, socializing, and self-care. For older adults, managing such pain is crucial for maintaining independence.
Temperature extremes affect the body in various ways. Cold weather can increase pain sensitivity and stiffen joints, particularly for those with arthritis. On the other hand, heat can lead to dehydration, disrupted sleep, and reduced physical activity, all of which can exacerbate pain.
One of the most dangerous aspects of heat waves is the lack of cooling at night, which prevents the body from recovering. This continuous stress can worsen existing pain conditions.
Key findings from the study
The researchers analyzed temperature data from different regions, defining extreme heat and cold based on local climate norms. For instance, a day reaching 84°F (28.9°C) in Maine was considered extreme, while in Houston it would take 96°F (35.4°C) to reach the same threshold.
Two significant patterns emerged from the data. First, individuals exposed to many extreme-cold days reported higher instances of high-impact pain. However, those facing occasional cold spells did not show the same effect, suggesting that the body can adapt to short-term cold but struggles with prolonged exposure.
Second, the impact of extreme heat varied by geography and socioeconomic status. Urban residents did not show a link between sustained heat and new pain, but rural residents did. This disparity is likely due to differences in outdoor work exposure, home insulation, and access to cooling centers.
Among the poorest older adults, sustained heat was associated with a higher likelihood of developing high-impact pain. In contrast, wealthier individuals, who have better access to air conditioning and healthcare, did not show the same correlation.
Lessons for improving public health responses
The findings highlight the need to include chronic pain in public health advisories related to extreme weather. State and county heat plans often focus on cardiac and respiratory patients but overlook those whose independence depends on managing pain.
Cooling assistance programs should be tailored to rural needs, addressing issues like home weatherization, energy assistance, and local delivery channels. Additionally, healthcare providers should consider a patient’s housing and local climate details when managing chronic pain.
With rising global temperatures, the risk of prolonged exposure to extreme heat is increasing.
